In the scene, Larys — who has the exacting nickname Clubfoot since he strolls with a limp because of a foot that was wound upon entering the world — makes one of his obscure visits to the Sovereign to share a few mysteries in return for favors. Also, we realize what one of those favors is as Alicent gradually strips off her stockings and puts her exposed feet on the table.

When Larys shares more subtleties, the imperial lays her feet on a seat and turns away.
“[He’s] taking a gander at her feet and afterward having a wank, fundamentally,” as Kilner expressively portrays it.
Kilner said the second was the brainchild of Sara Hess, a leader maker on the series and essayist on the episode.
